We will enter your new patient information into our system during your first appointment. We have forms for you to fill out in our office, but please feel free to download the paperwork below and fill it out before you come in. We recommend you come in 15 minutes early to complete all the paperwork if you don’t do it at home. If you have insurance, please ensure you have your provider and membership information when you come in.
You’ll receive a comprehensive oral exam during your first visit with Millennium Family Dental. We will talk with you about your dental history, problems you might be having, and your oral health goals. We will take a series of x-rays to see your entire mouth and any potential problems. The dentist will also perform an oral exam to check your gums and teeth for decay and existing fillings or crowns. Depending on the situation, you may receive a teeth cleaning during your first visit, or you might receive a cleaning during a follow-up visit. This will depend on the health of your teeth. Then, we will work with you to develop a personal treatment plan to help you maintain a beautiful, healthy smile. You can schedule a follow-up appointment when you leave to keep you on track for a healthy mouth.
